Supply and delivery of 74 cavans of Class A, Freshly-milled rice for the 2019 4th Qtr Rice Grant SSS OVP Luzon North 1 Division, inclusive of the March to December 2019 rice grant of the newly hired employees.

Price quotation must be inclusive of EWT
Approved Budget of the Contract (ABC): Php 185,000.00
Delivery commitment: 15 calendar days upon receipt of Purchase Order/Job Order/Letter Order

Submit the following in an envelope to the Administrative and General Accounting Section of the Office of the Vice President for Luzon North 1 Division or to the BACD Secretariat, 2nd Flr. SSS Bldg.,Harrison Rd., Baguio City:

A. COMPLETE supplier requirements:
1. Current Mayor’s/Business Permit
2. Certificate of Registration with PhilGEPS
3. BIR form 2303/Tax Clearance (BIR)
4. DTI/SEC/CDA whichever is applicable
5. Omnibus Sworn Statement

B. Fully accomplished/filled-in RFQ

Note: 1. Use long brown envelope.
2. Indicate your Business Name, Business Address and Notice Title in front of the envelope.
3. Affix signature on the flap of the SEALED envelope

C. Fully accomplished/conformed Technical Specifications

D. at lease 1/2 kilo of rice sample
