Description
|
INCLUSIONS:
1) The bidders shall submit the following Eligibility Documents in addition to the prescribed quotation form: (each page must be signed)
(i) Registration certificate from the Securities and Exchange Commission (SEC) for corporations, Department of Trade and Industry (DTI) for sole proprietorship, or Cooperative Development Authority (CDA) for cooperatives, whichever is applicable;
(ii) Valid Mayor’s permit issued by the city or municipality where the principal place of business of the prospective bidder is located;
(iii) Certificate of Registration from the BIR
(iv) Latest Income Tax Return with Financial Statements.
(v) PhilGEPS Certificate.
(vi) Bid docs requirements - available upon request
2) All entries shall be typed or written in a clearly legible manner.
3) Prices are inclusive of taxes.
4) Project site is at Municipal Hall, Baungon, Bukidnon.
